Am I Required To Sign A Letter Of 30 Day Termination If I Do Not Agree With The Assessment?

Am I required to sign a letter of 30 day termination if I do not agree with the assessment? I have been told by my boss and HR Director that I have to sign this letter which states job performance comments I wholeheartedly disagree with and know are personally motivated.
